Quick overview of projects we tackled in 2025:

  • FHIR Write - Notes: We expanded write-back capabilities, allowing external applications to write clinical notes (Text/PDF) back into the EHR. We defined two modes—Direct Write and Mediated Submission—to support diverse provider workflows.

  • Glucose Monitoring (CGM): Building on our previous work, we moved the needle from a community specification to a published standard. This facilitates the seamless flow of continuous glucose monitoring data into EHRs for better collaborative care.

  • Subscriptions Mini: We focused on the “Patient Data Feed,” creating a server-driven workflow to replace polling. We successfully tested event-driven and data-driven triggers to keep apps up-to-date with real-time data.

  • US Core & USCDI v6: We led design sprints to incorporate USCDI v6 requirements (like Facility Address and Medical Devices) into US Core, ensuring the implementation guide keeps pace with national regulations.

  • Granular Scopes Update: We addressed implementation challenges for granular scopes, drafting best practices for both server and app developers to ensure secure, specific data access.

  • R4 to R6 Transition Strategy: We took a forward-looking approach, developing a comprehensive strategy and migration principles to guide the US realm from FHIR R4 to R6 in the coming years.

  • Bulk Data Evolution: A massive year for Bulk! We advanced Bulk Data v3 (formalizing type filters), introduced Bulk Submit (formerly Import) for ingesting large datasets for quality measures, and piloted Bulk Publish for efficient hosting of static data like provider directories.
